NAION and vaccinations

 

Hello everyone,
I had my NAION ?event May of 2022, and after many very difficult months I am starting to settle into my new norm.?
My GP has recommended getting the shingles vaccine and when I questioned the possibility of an adverse reaction, he said it could be worse if I got shingles and affected my one good eye. Has anyone gotten the
shingles vaccination and how did you fair?

Sorry you went through all that...sounds exhausting.? I don't have much insight for you though..."lucky" for me I've not had much change in vision (or lack thereof) in the 3+ years since my incident. Had 2 covid vaccines, but stopped there as I got spooked.? ?Never been a big fan of doctors and meds, so I've never had a flu shot and haven't done the shingles thing either... but I've flown 3 times since the incident and that went well... anyhow, I'm sure none of that is helping, but I wanted to acknowledge your email...

The only reasons I can discern as the cause of my NAION in left eye? August 2022 are stress and anemia, (also that infamous low disc cup ratio thing),? although my iron levels were at the lowest end of normal, I had so many symptoms.? So after 9 weeks of high dose steroids, crazy no sleep, temporal artery biopsy (negative for giant cell), I got pneumonia for the first time in my life and was hospitalized and got to have more steroids... ugg. After about 8 weeks of recovery and having to miss my covid vaccine....yeah, got covid.? Finally around January, started feeling almost normal, almost back to normal energy levels....sleep still really elusive.

Hi Chiarina. Any change to your vision at the moment will cause you to worry. I remember waking up most mornings thinking my eyesight had deteriorated. This will stop as your eyesight stabilises. I agree it can be scary regarding information. It took me 3 weeks before I even wanted to know the term for what happened to my eye and just now started conversing about it. Illnesses can be scary.! I even told the drs to keep it simple at first they were giving me too much info that I was making my anxiety worse. We already had plans to drive to our destination anyway so I won’t have to deal with the plane now anyway. Thank you for writing??

Hi Lisa.? I am a traveller and I live in Australia so flights from here to anywhere take a lot of time.? Last year, I flew to Europe via Dubai and had no problems.? I asked my eye specialist if it was okay to fly long distance and he said it was fine.? The only issue I had was seeing the seat numbers on the plane so I just asked the flight attendant to take me to my seat.? I also have sleep Apnea so I brought my machine with me.? Whilst I didn't use it on board the flight, I saw a man on the same flight that had one and he was moved to another area of the plane where he could use it.??
I have low vision in both my eyes and I think the doctors are always reminding me to wear glasses when I am outdoors and just to be careful generally especially around my cat and dog and busy toddlers who may accidentally?bump or scratch the eye.? You just have to be careful. P.S A flight to Europe from Australia is 22 hours which includes transit and a flight to the USA is 12 to 14 hours.??

? I just wanted to describe my wife's experience and how it is somewhat similar to yours.
About a week or two previous we had to put down our much-loved dog due to kidney failure. Then she had to have a breast biopsy and xray scan which took about a half an hour. She was very stressed out due to both of these situations.
During the biopsy she had to lay on her left side on a hard table and was told not to move. She was very uncomfortable but could not move. Two nights later she suffered NAION in her left eye. Initially no one knew what it was. The first optometrist just Googled her symptoms to find out more. 2 weeks later she got to see a neuro opthologist who spent another 10 days sending her for every test in the book to rule out everything else. No sterioids by then.
?? So Stress plus pressure on the eye plus it was found she has "disc at risk" in both eyes. This was 5 years ago. Little change but some improvement since then.

Something upset the eye’s autoregulation, I will never know for sure.? I suspect it was a combination of many factors that led to my NAION episode, a "perfect storm" so to speak.? I too suspect there is a relationship between have migraine HA and NAION, perhaps it sets us up for developing NAION (along with our crowded eye structure).? Hopefully there will be more research in this area.???

Thank you all for sharing your experiences with longer air flights, all the tips and for the links Adam.? Good to know about people using their CPAP machines on the flights as it's been a while since I've taken a red eye overseas flight.? (I find out if I have sleep apnea on Wednesday.)?
Agree with Gary that all the unknowns with NAION make this an especially tough diagnosis.? Thx again.??

For what it is worth my NO said to stay hydrated ad don't sleep on the plane.? That would be hard on a long flight.? My sister who flies long distances a lot says there are a lot of people on her flights with portable CPAP machines sleeping away.

I don’t think there is good enough information to know for sure about flying—everything is anecdotal, and at best based on trying to be sensible about avoiding drops in blood flow/bp and oxygen.

?

In addition, it is known that high altitude (eg. Himalayas) ?climbers get optic disc swelling and neuropathy, so the advice makes at least some sense; ?see for example:? ;? ?

?

A modern jet has similar cabin pressure to an altitude of 8000 feet. However, it is known that oxygen saturation levels drop even in modern aircraft. See ? and go to the section on “Significantly Altered Physiology during Airline Flights”.

?

I personally have flown a number of times since I got NAION, including from the US East Coast to Hawaii overnight, even after I knew about the paper above, and I will be flying again. ?But I personally would not go hiking above 8,000 feet or so, out of caution. But other people will come to a different conclusion given the same (fundamentally unknown) risks.

?

Some other things I have heard to be careful of with NAION are weightlifting and playing woodwind and some brass instruments: Both temporarily increase intraocular pressure, which my NO said to avoid especially in the 8 weeks or so after I got NAION (ie, no pushups, weightlifting, sit-ups, lifting more than 30 lbs). But again, this was just his professional opinion, and not based on any rigorous study.

Thanks for the information on noises and flashing light in the affected eye.? I had noticed that right after my event (11/4/22) but it seems to not happen as often now.? Figured it was part of NAION but good to confirm.?
I do not know why snorkeling would be contraindicated so cannot answer that question.??

Regarding flying.? I did fly a week before my episode; short hops of 1 ? and 2 ? hours. (Nothing I have not done many times before).? I will be flying to Europe this fall and one flight will be 9 ? hours long.?

I saw Karl’s link from July 21 about flying and I am a little nervous now about my good eye.? I did a quick internet search and the link below is from a 2022 eyewiki article about airflight.??
Has anyone taken long flights after their episode?? I’ve been on flights of 8-9 hours before my NAION episode and tolerated them well.??This trip was postponed due to the pandemic and I really would like to go on the trip.? I?plan to talk to my MD but thought I would reach out for other’s experiences.? Thanks

Curious why snorkeling would be out? I have family that lives in Hawaii and visit often. Since my event I have snorkeled 10-12 times (over six visits). Not for long periods of time; perhaps an hour at longest.?

I also had the bright flash of light with loud noises (one night even when my husband closed the magnetic cover on his iPad). ?It had seemed to go away but recently has happened again. My event was in 2020.?
