Have you checked that +/- 5 v is really there? or is it just close? I use powered hubs that ensures that my voltage is true and also allows me additional devices such as a 2tb HDD for my Raspbery Pi.

Frank, It's never too late to get it right. You need to start again. Don't use Paged Mode (despite bad such advice on some OEM data sheets). If your system supports it, use Direct Mode. The difference
