I ended up opening up the speaker mic and doing some continuity checks to figure out what it does with the TRRS. TR goes to SP+ and RS goes to SP-.
Also tested the W2ENY adapter. Similar results

Yep,
As Andrey and Kevin say, the speaker outputs must never be grounded. Any load must be placed between the speaker+ and - connections. Many headsets will have their earpieces and microphone sharing

Tx500 audio out has spk+ and spk- wires and must be isolated from ground. With standard earphones/mic and adapters you connect it to ground.
