fltk 1.3.8 is what's on my Pi. I really need to upgrade the OS before continuing to troubleshoot this, Buster is just too old to be fussing with.

In port audio, I am used to making really specific selections in that menu. Leaving the capture and playback menu as default sounds like it would cause issues.

What about purging that version and downloading whatever version is in the repository? It couldn't hurt anything to try since your system's not working anyway.

Make sure Soundcard/Devices (Configuration menu) is set to PortAudio and Capture/Playback set correctly. If you're using a radio like the IC-7300, with built-in 'soundcard' then select USB Audio CODEC
