Maybe use a different e-mail client to see if the issue persists.
Alternatively, don't run any e-mail client at all temporarily and access the Apple-issued e-mail address using webmail. If you see anything in the spam folder with webmail, it is the mail server spam detection settings.

That would almost certainly prove that the problem is with the Apple Mail client app and not the Apple iCloud mail server.
