I have seen a similar issue with an iTouch on the latest iOS and the previous iOS. It did the same thing on a AV2 and a AV3.
Any time the processor would reboot (program upload) or lose power, the iTouch would go into the reconnection mode and never connect. You would have to close the app down and when it opened again it would reconnect fine as well. I too was trying to use guided access mode but couldn't because of this. So right now anytime I upload a program I need to find the 4 iTouches and close the app and relaunch it.
Using my iPhone 4s on the same iOS and the same Crestron app version everything would work fine and it would reconnect with no issues.
