One of the members on an IO group where I am a moderator had a Yahoo
email address. ... She kept getting a notice that she was not a member
of the group.

Most likely she was unsubscribed for marking a message as spam. Even if she herself didn't do it. Yahoo Mail is one of the services known to send such reports to Groups.io.

Have a look in your group's activity log to confirm this.

So she rejoined both groups using an AOL email address and all is
working well right now.

Out of the frying pan, into the fire. AOL started this mess, at least as far as Groups.io is concerned.

I know the whole Yahoo system has been erratic lately and I wonder if
that was the cause.

Perhaps indirectly, if the erratic behavior extends to diverting more messages than usual into users' Spam folders. But otherwise no.
