If you're looking for someone to make the PTO form because you don't have a 3D printer try JLPCB ( ), I had 4 made for me for $3.96 + $3.69 ($8.65 total) shipping by Global Direct to Wisconsin, USA. They look just like in the attachment except white (PLA resin). I'm sure there are others out there in cyber space and maybe even some libraries can make them for you. I'm not affiliated with JLPCB; I just use them to make PCB's occasionally when I can't mill them myself.

From the McMaster catalog (and as measured) - standard 3/4 x 20 nut is 7/16" in width (11.1125 mm) the hole as measured in Fusion 360 is 10.053 mm.? Therefore the hole is too small for the nut.? And since the PLA tends tends to change dimensions slightly as it cools I would recommend increasing the size of the hole to something like 11.15 mm.? If it ends up a little loose a drop of super glue will suffice.??
For another project I used melt-in nut inserts and with those and that worked fine but difficult for this project.
