The fan/s should be at the top blowing out.? Hot air rises so it will pull cooler air over the coils and exhaust it outward.? At least that's how mine was from the factory.

The tricky part is getting a fuse holder that is a light enough gauge.? I'm not sure you can stick a 12 gauge wire into the terminal block along with the existing wire.? You also can't splice in a

It should run regardless of which mode you are in.? The 12 volts comes from the power converter.? An in line fuse holder should be easy to find in an auto parts store.

You don't absolutely need a fuse, but it's a good idea.? There already is a fuse to your fridge, and the power for your fan does go through that fuse.? But if your fan shorts out, and it blows the

Those piggyback wire splices have a very small contact point and aren't that reliable.? They are especially prone to failure if they're exposed to water, and a little water does get through the vent
