I've done this for friends before. I've found pictures of a clock (time of birth), scales (weight), stork (date) and used the built in fonts on my machine to write the actual information. Then used a nice font (Hooked on Embroidery is a good place to look for great fonts!!!!) for the baby's name at the top.
You could add an appropriate picture at the bottom to finish it off.
All in all, much more fun and personal than finding a ready-done sampler design.
