Just my two cents, but I would think doing it as a badge would make each blanket have a stiff, not-so-comfy spot where the tag is.
I would do them as a quilt label - just use a good linen or muslin, and perhaps put a simple floral or other border around the writing. Be sure to leave space between each on to give them room to turn edges under or stitch in some way to keep the labels from fraying.

Once again I'm asking fellow embroideriers for their help..I have been
