I got it working, but I did notice something strange in QLog. These are the steps I took:
1. Assign a Profile Name for the rotator. Ex. Yaesu G-1000 DXC PST"
2. Select the "hamlib" interface
3. Ignore "Rot Model" section
3. Select port type "Network"
5. Enter the host name. Ex 127.0.0.1
6. Enter the port number Ex 12000
7. Return to "Interface" and select "PSTRotator"
8. "Rot Model" will default to "Rot 1"
9. "Port Type" becomes greyed out and can not be modified
10. Click Modify
11 Click Save
Selecting "hamlib" will allow "Port Type" to be modified but selecting PSTRotator prevents modification. There is no way to change from Network to Serial or Serial to Network when PSTRotator is selected first.
Selecting PSTRotator does not default to "Network" automatically and will remain on "Serial" if that is what is displayed from the start.
