Sorry about this, but I made an error when I said that the binary input pins of the 74141 are 2,3,6,7. They are not 2,3,6,7 !!! Consulting the data sheet, they are actually 3,4,6,7 Where Q1 = pin 3 Q2

The 74141 (like all DIP ics) has a notch in the center of the package at the "top", which is between pins 1 and 16. Some ics have a dot near pin 1. With the ic facing so that the pins are pointing

First of all, simplify your circuit for troubleshooting. Get that stamp out of there, and use four plain SPST dip switches with 2.2K pullup resistors on each of the binary input pins of the 74141 to
