I think the small black removeable gear wheel comes with the fine feed, the handle comes with the mill. So someone somewhere is missing the gear wheel for their fine feed.

Then the two pieces bolt on to the side of the milling head to provide the fine feed, like this:
Loosening the bolt allows rear of the silver portion to move upwards, which disengages the fine feed

Quill Feed Gear?
I bought this Unimat 3 quill handle and it has this gear on it. Is this a Unimat item for a quill feed drive, or possibly something someone made?

Note the reason for bushing out the head stock as well was to enable me to bore out the carriage from through the headstock leadscrew hole. I moved the carriage up as close to the headstock as I could
