Can anyone explain the mechanism of how a pilot is hit in a multi engine plane?

This will sound a strange thing to come out for such an old ruleset, but it is something I recently noticed that I cannot see how you get a pilot hit on the damage charts nor could I find anything about rolling for it on a cockpit hit.
