If your incorrect address of 4 instead of 14 was written in hex or octal rather than decimal, I'd suspect a bad DIP switch. But IEEE-488 addresses are usually stated in decimal, and context in the

Adam, Could the transistors be bad? The 2N5179 replaces ones on this board of the p/n, 151-0282-00. If you have component cooler, use that to briefly spray on the transistors to see if cooling them
