The RX/TX audio streams between iPhone and QDX/QMX are always purely digital via the virtual USB sound card that's implemented by QDX/QMX firmware. So you won't be able to hear anything. The VOX here is about TX and it's on QDX. What it means is that with VOX enabled, if QDX detects the USB host (iPhone or PC) is sending an audio stream to it, it will start to generate FSK from the audio stream. I'd suggest review the "Design" section in the QDX assembly manual.

As for the troubleshooting, I'd suggest using your PC as a mock iPhone to test your QDX. Below are the steps.
  1. Base a baseline, make sure your QDX is fully working with your PC.
  2. Functionality wise, a PC is no different from an iPhone, with the exception of CAT control. In your WSJT-x configuration, disable CAT for the radio completely. Without CAT control for band/freq and PTT, your WSJT-x behaves exactly like iFTx on your iPhone. Now the only things you still have is the digital audio input and output streams via the virtual USB sound card.
  3. Launch the terminal application to configure the default frequency and turn on the VOX.
  4. Success check: after power cycle your QDX and restart WSJT-x, you should still be able to receive and transmit FT8 just like before. You won't be able to change band/freq though because CAT is disabled.
  5. If step 4 is successful, you can be sure that
    • You QDX is working fine on both RX and TX
    • It has also been configured for iFTx properly
    • The USB cable is now known good
  6. Connect a USB keyboard to your iPhone with that Lightning to USB Camera adapter. You should be able to use the USB keyboard to type on iPhone now. If not, that adapter is not working properly.
  7. Now connect your QDX using the known good USB cable from steps 1 to 4 and launch iFTx. I don't really see how things can fail at this point, if iPhone is unmuted and the output is set to max.
will it be enough to make a cable between the OTG cable and the Qmx?
I don't quite get what does this "make a cable" mean. It should be a normal USB-A to USB-C cable. There's no analog audio involved, so there's no need for any special cable. Good luck with your QDX and QMX!
