Hello Joan,

When I first tested the 0_16, I encountered roughly the same issue as you, and I proceeded to do the same routine when dealing with keying problems: I cleaned the contacts with 800 grit sandpaper and tested against my IC-7300.
Well, unlike the other glitches, the problem was manifesting on the 7300 as well so at that moment I knew it wasn't the QMX. Next, I replaced the paddle with a HM one (from a bipolar relay) and all was perfect. So, my original paddle was the culprit, but how? Finally, I loosened the burried bolts that act as pivots (and electrical contact of the paddle with the base, which in turn is the ground), then I screwed them back in place. I gave it another try and, lo and behold, no more pain :-)

So, I don't know if that's the answer in your case as well, but you might want to check how the paddle connects with the ground and see if it isn't some intermittent contact.
