I'm pretty sure I've located a 5082-7415, or at least a display that's very close. Strangely, in Radio Shack packaging:
The pinout and general characteristics are the same as the HP 5082-7415, the back of the card says "TYPE 5082", and Tandy was selling displays from HP, Monsanto, National Semiconductor, and just about all of the other LED manufacturers back then.
