Groundhog Day. This is the number one question on GMF with all the new members, by my non-scientific survey. You can use a pending subscription notice (settings¡ª> member notices¡ª> create new notice ¡ª> choose type ¡®pending subscription¡¯) and put your questionnaire in it. They return it by email and their response goes to the lender address. But it can be wonky and be lost on either side. I¡¯m constantly urging Mark on beta to implement at least a text box. You might want to add to my pleas ?there as well.
