In my case if was a multitude of problems like layout and wrong toroids.

You are seeing those strong spurious and high power. Check output filter
components for wrong values or failed parts.

How did you troubleshoot it? Was it a bad/wrong resistor or cap - or just an over-enthusiastic transistor?
