It may be because it takes more chemicals and more time to do a complete removal of excess copper.? Outline or isolation is faster and less expensive.?
At least that works for me when doing CNC Milling of a PCB.? It would take
forever to machine excess copper off with the tiny bits we use for CNC mills.

Bottom side is sufficient. With lots of topside copper, more chance of shorts if solder mask isn't perfect. A topside groundplane would be mostly cut up into long skinny regions that wouldn't do much good.
