Just to put thr record straight, once we figured out which batch was the problem, we did not proceed with it. We junked 500 of the remaining chips, tried many other sources and found good ones.?
That said, it did take us time to realize there was a problem. Unless somethinf emerges as a pattern, it becomes difficult to spot a production failure. We cannot halt production for jndividually reported problems.
To be on the safe side, we now subject every board to 14v to smoke out any potential problems.

Just make sure you've got everything plugged in all the way before powering on. Double check your wiring on the audio pot to make sure it's not flipped. With the headphones - do not plug in when powered on or pull out to be safe.? I've tracked down a replacement ic for the TDA2822 already and have a 386 I'm going to breadboard in the meantime.?
