The drop in gain above 2.2 GHz is very consistent and has fully been compensated in the factory calibration.
Above 6 GHz the ZS405/406 LNA is useless and better be disabled

Is it a computer that has both USB2 and USB3 ports?
If yes try a computer that has only USB2
There is a known incompatibility with USB3, check the WIKI and the user group

The error I get on my laptop is "Unknown USB Device (Device Descriptor Request Failed)". It seems I must have a defective/failed USB port on the SA. The SA works well in every other respect.

Most of the attempts are 2.0. The primary connection I am using is 2.0. During my "various" ports attempts I did try a 3.0.

Changed out U14 LNA and Tests 10 and 14 are now good - only Test 13 fails.
Is this Test 13 failure from U22 being bad?
I also bought a few of them when I got the LNA chips
