Actually DTMF is used only via the local exchange or Central Office. On toll trunks, MF tones are used.. The big difference is DTMF is not harmonically related whereas the MF tones are. MF signaling

The + indicates the international call access code, which, as David indicated, is 011 in the US.? Some other countries might have a different access code, so they used + to "use your local access

That is? E.164 formatting.?? Numbers in the North American Numbering Plan? (NANP) start with +1 , followed by the are code and number.
