George having experience with shipping stuff to UK and also Europe my guess it should run around $80 to $90 for USPS international Priority Mail depending on the size of the box, weight is negligible

The instrument has a damaged CRT, most likely from being dropped, or otherwise impacted. I have seen a few CRT's with similar damage over the years. No solution other than replacement.
