I have rev D, but didn't upload it because you normally have to purchase this document. Since it is an official specification, I believe it should be freely available, but that's not the way it is. When even laws are locked up behind a paywall, what are you going to do?
For the nitty gritty details, the TIA-603 spec is the document to read. The Harris document simplifies things, and gives you a step-by-step procedure.
BTW, the Harris document was originally obtained from their website, so there shouldn't be any problem distributing it. I even have a hard copy that they gave me years ago.
