Maybe oscillating, but I really don't understand this one. ?My impression is he's not hitting the PTT button, just touching the heat-sink with his finger. ?So no 12v TX rail into the 5v regulator

It is not going into transmit, and probably not into avalanche.
It is probably going into self-oscillation. You may need to install
a 10 ohm resistor in the lead right next to the gate.

It may not be obvious that the balanced modulator at D15,D16 is multiplying anything. ?Consider this: ?When the 12mhz bfo is on the positive side of its cycle, both diodes are fully conducting.
