Attention Parents!

If your children are attending public school anywhere in Canada or the USA, please complete and submit the following Sensitive Curriculum Information Request Form to your child's school.

Make sure to print a copy for each child, check all applicable boxes, sign, and submit the form. Follow up with the school to ensure its implementation.
